Yellow House partners with residential, PHP, and IOP programs to help eligible clients establish outpatient psychiatric care before discharge. The relationship can begin while the treatment team is still involved and continue by telehealth after the client returns home anywhere in California.

The goal is not simply to add another provider name to a discharge packet. It is to begin the outpatient relationship while the client still has support around them.

One California wide option

Telehealth allows eligible clients to continue with Yellow House after returning home throughout California. Your staff does not need to build a different psychiatric referral list for every community.

More reliable access

Partner referrals receive a direct scheduling pathway and prioritized coordination based on discharge timing, current availability, and clinical appropriateness. New clients can generally be seen within 7 to 10 days.

Medication continuity

Discharge medications, recent changes, pharmacy information, response, side effects, and follow up timing are reviewed so the family has a clear outpatient medication plan.

Consent based communication

With the necessary permissions, Yellow House can coordinate with the treatment team, review recommendations, and communicate relevant concerns during the transition.

A clearer escalation pathway

If outpatient care appears insufficient, Yellow House can help the family identify the need for reassessment and, with permission, communicate with the program or designated contact.

How the Yellow House Continuity Partnership works

A clear seven step process, beginning before discharge and continuing into outpatient care.

The program identifies an appropriate client

The client is a California resident between ages 10 and 25, is clinically appropriate for outpatient telehealth psychiatry, and the client or family agrees to the referral.

A warm referral is submitted

Using a secure process, the program shares the anticipated transition date, diagnoses, medications, current level of care, and relevant clinical contacts.

Fit and availability are confirmed

Yellow House reviews whether the case is within scope and whether appropriate scheduling is available. A referral does not guarantee acceptance, and clinical fit always comes first.

The first appointment is secured

Whenever possible, the appointment is booked before discharge. Care may begin while the client is participating in IOP if timing and clinical circumstances support it.

The clinical handoff occurs

With proper authorization, records are transferred and Yellow House coordinates with the program prescriber, therapist, or other designated clinician.

Yellow House continues outpatient care

Yellow House provides psychiatric assessment, diagnostic clarification, medication management, and coordination with the client’s ongoing therapist.

Relevant concerns are communicated

With the necessary permissions, meaningful deterioration or a possible need for a higher level of care can be communicated to the designated program contact.

Medication continuity and reconciliation

Clients often leave structured treatment after several medication changes and with a limited medication supply. The transition process creates a clear picture of what the client is taking, what changed, and what needs to happen next.

Review discharge medications and recent changes
Reconcile medication bottles, prescriptions, and discharge instructions
Confirm the client’s preferred pharmacy
Clarify what was started, discontinued, or adjusted
Create a plan for monitoring response and adverse effects
Schedule follow up before the current medication supply runs out whenever possible

Referral fit and important limits

The partnership is designed to make appropriate transitions easier. It does not mean every referral will automatically be accepted.

  • •Current psychiatric emergency or acute safety concern
  • •Needs a higher level of care or in person monitoring
  • •Primary substance use treatment needs
  • •First episode psychosis that has not been stabilized
  • •Clinical needs outside the scope of Yellow House
Private pay: The initial psychiatric evaluation is $450 and follow up appointments are $225. Yellow House does not bill insurance directly. A superbill is provided for families who wish to seek out of network reimbursement.
